Download TouchWiz 5.0 Theme for Samsung Galaxy Y
Here is a TouchWiz 5.0 Theme by marinalin85. A quite talented themer who has so far made a Jelly Bean theme, LG UI theme and this TouchWiz 5.0 theme for the Galaxy Y.
According to his thread, this theme contains modifications done to the dialer, contacts, framework-res and systemUI apk's.

Instructions:
Boot into Recovery mode by holding Volume up + Home + Power
Choose to apply update from sdcard and flash CWM recovery - Download CWM.zip
Once in CWM Recovery, select to install zip from sdcard
Find the theme zip file which you just downloaded and choose it
Choose yes
After installation is done, reboot
